The fee for endodontic therapy is determined by the complexity of the tooth being treated. If endodontic treatment is indicated and decided upon, a fee will be quoted. This fee includes: the consultation, all necessary radiographs (x-rays), all routine office visits needed during normal endodontic treatment, routine post-treatment recalls and checks. Any circumstances requiring additional procedures (i.e.: post and core build-ups, additional diagnostic appointments, microsurgery) or unforseen complications (i.e.: additional roots, calcified canals) or any other visits other than during normal office hours are subject to additional charges.
A late payment charge of 1.5% per month will be assessed on invoices outstanding 90 or more days from the date of treatment ($1.00 minimum). We will help provide and prepare our standard insurance forms and any necessary reports to assist you in making collection from your insurance company.
Please give at least 48 hours notice of change of appointment if cancellation is absolutely necessary, otherwise a charge may be made.
Lab Fees

If it is necessary for us to order laboratory tests such as a biopsy, you will be billed directly by them and responsible for payment of that bill.
