First Endodontic Visit

We respect that your time is valuable and strive to complete your treatment in one visit, always keeping in mind that we will never compromise quality of care.

All patient visits start with a comprehensive evaluation that includes a thorough review of your health history as well as an oral evaluation that includes several diagnostic tests that will help us determine your treatment needs.

Medical Alert

We require physician releases for some medical conditions.  Please alert the office at the time of scheduling your appointment if you have a medical condition that may be of concern prior to treatment such as:

  • Diabetes
  • High blood pressure
  • Artificial heart valves
  • Joint replacements
  • Active in chemotherapy or radiation treatment
  • Pregnancy
  • If you are on any medication (i.e.  Anticoagulant therapy, Bisphosphonate medications etc.)

Please assist us by providing the following information at the time of your consultation:

  • Please bring a list of ALL medications and herbal supplements you are currently taking
  • Please bring a copy of your referral slip
  • Please give us your insurance information at the time of scheduling your appointment
  • If possible please fill out your patient registration forms on-line (click here to fill out your registration forms)

If you have been advised by your physician or dentist to use antibiotic premedication because of mitral valve prolapse (MVP), heart murmur, hip, knee, cardiac or other prosthesis, please make sure you are on the appropriate antibiotic on the day of your appointment. If there is a question, please call our office prior to your appointment.

IMPORTANT:
All patients under the age of 18 years of age must be accompanied by a parent or guardian for all visits.

 

Please give at least 48 hours notice of change of appointment if cancellation is absolutely necessary, otherwise a charge may be made.
